Outpatient Diabetes Management and Diabetes Education
Diabetes is a complex condition requiring ongoing care management in an outpatient setting. Upon discharge, a referral can be made to 天涯社区官网 Medical Group - Endocrinology & Diabetes for diabetes management and/or diabetes education. To be seen, a referral is needed by your primary care provider.

The team also includes registered dietitians and registered nurses. Many of the team who provide nutritional counseling and diabetes education are also certified diabetes educators.
天涯社区官网 Endocrinology and Diabetes also offers free pre-diabetes classes. Additionally, nutritional counseling is available for diabetes, weight management, high cholesterol, gastrointestinal conditions and other various conditions.
